• 2 Periodic and 1 Emergency Exam
• 2 Routine Cleanings
• Bite-wing x-rays as necessary
• 2 Fluoride Treatments

(avg. of $150 savings on preventative treatment)

20% Discount on:

• Diagnostic and Preventative Services
• Fillings
• Root Canal Therapy
• Periodontics
• Crowns, Bridges, Dentures and Partials
• Oral Surgery

10% Discount

• Cosmetic Bonding
• Veneers

All annual memberships require payment in advanced. The program is effective from the date of membership purchase.
